Folate refers to a group of water-soluble B-vitamins which play a critical role in deoxyribonucleic acid (DNA) synthesis and methylation reactions [ 1]. All folate vitamers consist of a common parent structure, but differ based on whether the pteridine ring is in an oxidized or a reduced state, the addition of one-carbon substituents at positions N5 and/or N10 and number of glutamic acid residues [ 1, 2]. Food folates are reduced and have a polyglutamate tail consisting of up to nine glutamate residues [ 1, 3]. Folic acid is a synthetic form of folate which is oxidized and contains one glutamic acid residue; this is the most stable form. Intake of folic acid requires it to be converted to L-5-methyltetrahydrofolate for it to be used by the body. This conversion mainly occurs in the liver; however, not all of it may be converted, and some may enter the circulation as it is. As a result, the infants who take folic acid fortified milk are often found to have a high level of unmetabolized folic acid in their plasma, which on its own may not be of any use to the growing baby.

Most of us metabolize folate and folic acid and transform them to 5-MTHF efficiently. There is a gene that controls this process. It’s called MTHFR, and we each have two copies of it. However, a lot of us have variations of one or both of these gene copies that don’t allow the transformations to 5-MTHF. If one gene copy is bad for folate absorption, your efficiency may take a huge hit. If both copies are bad, you won’t metabolize much of folate or folic acid. A very serious allergic reaction to this drug is rare.
